Ship Survey & Inspection

Audits

•  The inspection and examination of a process or quality system to ensure compliance with requirements. An audit can apply to an entire organization or be specific to a function, process, or production step.
•  The objective of a company SMS & ISPS audit is to seek information about the Safety Management System and to determine (by objective evidence) the degree of compliance with the applicable rules, regulation and standards (ISM, ISO etc), and the documented company procedures.



Navigation Audit

•  Navigational audits are a requirement of TMSA and becoming more and more common in other trades e.g. bulk carriers and other vessels.
•  The biggest risk facing a ship owner is a major navigational incident in spite of the fact that the techniques for safe navigation are well known. When these techniques are professionally executed, navigational risks are reduced.



ISM/MLC/ISPS Internal Audit

•  The International Safety Management Code (ISM) require Shipping Companies to have procedures for conducting both internal and external audits to ensure the ship as well as the shore organisation is doing what is documented in the Safety Management System.
•  The International Ship and Port Facility Security Code (ISPS) require Shipping Companies to have procedures for conducting both internal and external audits to ensure the ship is doing what is documented in the Ship Security Plan.



Environmental Compliance Audit

•  The Environmental Audits cover all applicable annexes of the Marpol, as implemented through the Safety Management Systems. The audits are detailed and cover physical condition of various Marpol equipment, level of implementation of procedures, accuracy of records, review of crew familiarity with the requirements.

Surveys

•  Surveys are conducted onboard a ship to check the strength, safety equipments and to check whether the ship is operational for voyage or not.
•  Survey improves the ship safety and it also provides safety to seafarers working on ship. It is conducted by a qualified Marine Surveyor.



Pre-Purchase Survey

•  This type of survey carried out in ships sale/purchase transactions where time is a very limiting constraint and offers are valid for a short period only.
•  In such a case a so-called “Superficial Survey” to evaluate the condition of the ship and its associated machinery, has often to be performed at very short notice. This survey based mainly on visual observations of parts of the ship during a relatively rapid tour.
•  The superficial condition survey will follow the ‘international institute of marine surveyors’ guidelines enabling the customer to formulate his own opinion regarding the condition of the vessel, over and above that of the opinion reflected by the inspector, based on the approximately 50 page report and numerous additional photographs and photocopies supplied.



Ship Condition Survey

•  SOS Indi Inspection & Consultant organises condition surveys to enable managers to determine whether entered ships conform to acceptable standards.
•  SOS Indi Inspection & Consultant provides a comprehensive survey to ascertain the physical condition of the vessel and the effectiveness of the onboard management. If deficiencies are found, the surveyor will provide the Master and/or owners’ representative with a list of items in need of rectification.

Inspection

•  Ship inspections comprise of a thorough visual check on the vessel's physical structure and condition, testing of critical machinery and safety equipment, as well as a comprehensive review of its documentation and certificate history.



Pre- Vetting Inspections

•  The purpose of these inspections is to successfully complete Cargo Major Inspections. Cargo Majors or Petroleum Terminals generally conduct an inspection of the 3rd party vessels against standards of safe vessel operation developed through inputs of various stakeholders in the industry.
•  During a Pre-vetting Inspection we visit the vessel and perform an in-depth inspection covering all areas on board in relation to the SIRE Vetting programme with special focus on the Vessel Inspection Questionnaire (VIQ) in order to prepare the vessel for an upcoming vetting.



SIRE-OCMF

•  One of the most significant safety initiatives introduced by OCIMF is the Ship Inspection Report Programme (SIRE). This programme was originally launched in 1993 to specifically address concerns about sub-standard shipping. The SIRE Programme is a unique tanker risk assessment tool of value to charterers, ship operators, terminal operators and government bodies concerned with ship safety.


•  The SIRE programme requires a uniform inspection protocol that is predicated by the following:
  1. Vessel Inspection Questionnaire (VIQ)
  2. Barges Inspection Questionnaire (BIQ)
  3. Uniform SIRE Inspection Report
  4. Vessels Particulars Questionnaire (VPQ)
  5. Barge Particulars Questionnaire (BPQ)

Investigation

•  Ship inspections comprise of a thorough visual check on the vessel's physical structure and condition, testing of critical machinery and safety equipment, as well as a comprehensive review of its documentation and certificate history.



Accident Investigation

•  An accident investigation might seem like a reactive tool — since an undesired event has to take place, or nearly take place, to spur an investigation — it’s not necessarily the case. A well-built investigation process can get to the root cause(s) of an event and prevent similar events from occurring again.

Incident Investigation

•  This incident investigation procedure provides an approach to determine initiating events, contributing events, root cause, and contributing causes. The investigation must identify appropriate recommendations that address the problems and identify root causes.
